As a USCIS Civil Surgeon (Immigration Doctor), Dr. Resil provides Physical Examination for the USCIS I-693 forms.  

Pricing

The cost for an adult between 25 and 44 years old is $490 without vaccines. 

Cost Breakdown:

• $200 - Physical Exam
• $190 - Tuberculosis (TB) Lab Test
• $100 Syphilis Lab Test
• = $490 total (without vaccines)

Required Documents

You will need to bring either a valid passport or a United States issued ID, Work Permit or Alien Registration Number (A-Number).  and any vaccinations or lab tests you've had done. All valid immunization records are accepted (even from abroad). 

* Please note that medical insurance does not cover anything regarding immigration medical exams as they do not consider it a medical necessity. However, you can use your insurance with your primary care doctor or at the pharmacy to get any vaccines you may be missing.

The Physical and Labs are REQUIRED to be performed by the Civil Surgeon

This includes the physical examination and the Quantiferon Tuberculosis (TB), the Syphilis, and the Gonorrhea lab tests. A comprehensive physical examination will be performed by Dr. Claude Resil (chaperoned by female medical staff when necessary) or by one of the employed Nurse Practitioners.

Required Vaccinations (Ages 18 - 64)

• Tetanus vaccine ($150)
• MMR vaccine ($160)
• Varicella vaccine ($220)
• Hepatitis B vaccine ($150)
• Polio vaccine ($130)
• Influenza vaccine (price varies during each flu season)

Required Vaccinations (Ages 65+)

• Tetanus vaccine ($150)
• Varicella vaccine ($220)
• Pneumococcal (Pneumonia) vaccine ($220)
• Polio vaccine ($130)

* For children: all scheduled vaccines are required for the appropriate age groups

Tuberculosis Blood Tests

Quantiferon or T-Spot blood test is required for those aged 2 or older. If the  Quantiferon/T-Spot result is positive, a chest X-ray is required to rule out the possibility of an active Pulmonary Tuberculosis infection. The X-ray can be done at a nearby hospital and interpreted within 1-2 hours due to our affiliation with the local hospitals (Beth Israel Hospital in Milton or Good Samaritan Medical Center in Brockton).

Beginning October 1st, 2018, the PPD Skin test is no longer an acceptable form of TB screening.

Additional testing and fees may be necessary, based on the individual’s medical condition.
